Dr. Braverman,
I have 2 beautiful children and never dreamed it would be so hard to have a third. I had 3 miscarriages in one year and still have no concrete reason for all these heartbreaking losses. First round of testing revealed abnormal PTT and slightly elevated anti-cardiolipin antibodies. Ob sent me to a hematologist who do further testing and all results were normal. Second round of testing revealed PAI 4g/4g. The hematologist said this used to be linked to multiple miscarriages but there is not sufficient research to back that up anymore. Someone else posted a question about this diagnosis and you mentioned that testing should be done for PCOS. I have no symptoms and chart my cycles so I know I am ovulating regularly. Would you still recommend testing for it? You also said PAI 4g/4g can cause clotting issues. Is there a way to tell if it is causing them before taking Lovenox? Also, if I have a clotting issue, can you explain why it didn't effect my first two full term pregnancies?

I am more inclined to look for the PCOS connection or another immune issue before a clotting issue. I agree with you that a polymorphism you were born with would have most likely affected one of your first two pregnancies. APA syndromes and PCOS can get worse over time, and those along with any new auto immune issue is most likely the cause of your isses.

Thank you for your reply. I just have a few more questions for you. From what I've read about PCOS, I have no symptoms. Wouldn't I have at least a few symptoms if that was a possible cause of my miscarriages? In my last pregnancy that ended in miscarriage I took a baby aspirin daily, which I did not do with the first two miscarriages. That pregnancy lasted longer and we actually saw the heartbeat, which also didn't happen with the other miscarriages. Is it possible that there is a clotting issue and the baby aspirin helped some but wasn't enough? I am planning to schedule a consultation with you as soon as I am financially able. If I had my records sent to you would you re-do all the testing I have already had done or would you be able to use that information?

yes its possible the baby aspirin only partially treated your condition. Yes at the time of the consult , I always go through what labs you have already had done and only order what I think you will need. I look forward to working with you.
