Dear doctor Braverman

I have an urgent question, it is regarding my pregnancy and the health of our baby.

Currently I am 31 weeks and 2 days pregnant and I am hospitalized since 2 days.
The reason for this is that the baby is behind in growth (growth retardation about 2,5 weeks behind). Also the blood flow through the placenta is worsening. The pressure is getting higher. The baby is being monitored and the heart is still okay also the baby is still moving enough and there is still enough water left.

I use 5 mg folic acid and 100 mg B6 this is for my homozygote MTHFR mutation 6776tt Before this pregnancy I have had 4 early miscarriages all at approx. 6 wks.

My doctors here in Europe do not think that taking heparine at this point in pregnancy will influence the situation. They tell me I would have need to take heparine at the start of pregnancy. Could you please give your opinion in this urgent matter or if you have any other advice for me at this point?

I dont think heparin at this point would be useful. Best right now is hydration, resting on your left side as much as possilbe. I dont think this is an MTHFR issue, more likley to be immunologic.
Just keep a close eye on your blood work such as liver enzymes and kidney fucnitons and platelets. when these values begin to change or if the babies testing begins to change that is when you should consider delivery. But for now observation and giving the baby more time to mature is the correct treatment. Also you should consider a course of steroids to assist in lung maturity in case there is an early delviery.
The good news is that babies that have intrauterine stress actually do better once delivered than a baby of the same gestational age that does not have this same stress.
