4th ICSI failure

Dear Dr Braverman

We have recently just experienced our 4th failed ICSI cycle (all in all we have had 7 embryos transferred over the 4 cycles out of which 4 of them were blastocysts). We had a chemical pregnancy on the second cycle but apart from that every single cycle has resulted in a negative test result.

My husband has severe oligospermia and after our 3rd failed cycle, I had a hysteroscopy which was normal and I also had immunology testing which revealed slightly abnormal results in the following:

Active protein C resistance - 1.29 (N Ratio) normal range 0.90-1.20
PAI-I 4G/5G - heterozygous mutated
NK assay CD56 - 15.5 (normal range 2-12)
and CD19 & CD5 - 2 (normal range 5-10)

We had two blastocysts transferred on our 4th cycle and I also had the immunlogy treatments - clexane, intralipids, steroids and baby aspirin.

What would your advice be as to our next steps?

Any help would be greatly appreciated.

NK cell elevation without knowing all of the subsets does not help in making the diagnosis of immune related rejection. If you have a full panel of immune cells performed I would need to see that to help you decide on correct treatment.
In odering these immune treatments the amount and frequency and even the correct treatments are based on the changing immune environment as pregnancy progresses.
