Dr. Braverman,

I just had a consult with a reproductive immunologist to get a second opinion before starting our third IVF attempt. I’m 43yrs and my husband 40, we married in our early 30s and 5 years ago we found out our diagnosis: male factor and now advanced maternal age. I had my first IVF cycle 12/2009 low response, 2day/2 embies transfer but no success. Six months later (after doing acupuncture/TCM/supplements) I got 11 eggs, 4 embies, 3day transfer, BFP but it ended at 5 weeks as a chemical pregnancy. For now, donor eggs/sperms are not an option for us.

The Dr told me that because of MF she would recommend a DNA fragmentation assay (he already had the karyotype test/ Normal) and an AMH (my FSH is 17) to check the probability of success of another cycle. She doesn’t think it’s an immune problem, but the quality of our sperm/eggs and to just try again. What do you think? Should we just go ahead and try one more time? Do just the suggested testing? Would you suggest something else?

Hi Jobeth,

There are so many issues that need to be deal with when performing IVF with AMA and MF. Both need to be optimized. We have special protocols we use such as estrogen priming, HGH, DHEA, supplements, and for the male factor DNA frag is a reasonable option. Its hard to say what the reason for your loss is, as AMA is most likely cuase but no way to know if immune issues playing a part without testing. The hard part is that if therre is an immune issue the good embryos will be few and far between and it makes sense to rule that out prior to trying another cycle. I would be happy to assist you with that if you need.
