Dear Dr. Braveman,

I am a 34 year old woman with one child (6yrs old). I have been trying to concieve another child for the past 5 1/2 years with no success. My husband and I began trying for our first baby in 2005. I had irregular periods (sometimes 100+ day cycles) and was diagnosed with PCOS (although I am thin and don't have insulin resistance). All other tests were normal. Before beginning fertility treatments I had an HSG which showed both tubes were blocked. I underwent LAP surgery, where both tubes were opened, a polyp removed, and three very small spots of endo were discovered (two of which were removed). In the years after the surgery I went through 3 clomid/IUI cycles and 2 inject/IUI cycles which failed depite responding well to the meds and my husband having excellent sperm samples. We then swithched to a private clinic to do IVF. Again, all routine tests came back normal except ones showing PCOS. Underwent two IVFs which both failed. Embroyos were fragmented both times (8 first cycle, 5 second cycle). Following my last failed IVF cycle, while deciding what to do next, (i.e. ZIFT, GIFT, IVF, etc.) I ovulated on cycle day 111 and miraculously concieved on my own. Carried my daughter to term with no major problems.
Fast forward to today. We have been trying naturally to have another baby. I do ovulate more often on my own in the last couple years (last year for example I had 9 cycles. Much better than 3-4 I had before getting pregnant) However, about three years ago I began to have hives the week leading up to ovulation and then again before menstration. Then, about a year and a half ago I got a positive ANA maker (Nucleolar 1:640). I can't help but feel I have some sort of immune issue going on. I have not had any miscarriages, I just do not get pregnant. I don't know where to start with testing. I live in California, but will be traveling to New York in July for a wedding. Any guidance is greatly appreciated.

You certainly do have an immune issue. I can help you with this. Please fill out a consult form on the website and we can schedule a free 10 minute chat to discuss your case. We can get all the testing done while you are still in California but I would recommend coming her to review with me(although if necessary we can discuss with a doctor in California).

Thank you so much for responding. Is the consultation form the same as the "patient questionaire"?
