Dr. Braverman,
We recently found out I'm about 5 weeks pregnant (on my Bday yea!). My HCG results this Monday were 2215; 48 hours later it was 4894. I'm taking baby aspirin an 200 mg of progesterone a day. I had a few results I wanted you to see:
antiphospholipid img: 26 mpl
antiphosphatidylglyerol igm: 18 mpl

MTHFR pos for heterozygous c677t neg for wildtype.
APTT (lupus sensitive) 36.6 low
DRVVT (complete) 48.9 high
Anti-Annexin A5 abs (igm) 17 high
Factor VII 136 high
Factor XIII screen 66 low
TNF-a:IL-10 (cd3+/cd4+) 46.2 high
IFN-g:IL-10 (cd3+/cd4+) 23.3 high
cd4 helper 64.0
cd19+/cd5+ 16.7 high


What do you make of that mess?

You clearly have an inflammatory condition the etiology is not certain but this requires treatment, its not just the TNF which we see elevated quite commonly , (yours is quite high however) but the INF gamma a more potent cytokine is elevated as well. Please call me right away I will squeeze you in as this should be dealt with immediately.

...I'm really scared. What types of treatments are there for this? pregnant and after pregnacy. What do you think of the factor VII and factor XIII abnormalities? Do you think any of these results will cause problems after pregnancy? Is it looking like the beginning of Lupus?

I'm spotting already...dark red. Not enough for a pad, it's still only when I wipe. No major cramping yet either, but since I've done this 5 times now, it seems like the start of another miscarriage. My husband is off to Afghanistan next month so guess I'm out of luck for another year. Should I stop taking my progesterone? It's like each miscarriage gets easier for my family to accept. However, it's not so with me. I feel like a 5 time failure. And I can't wait to hear everyone's comments about how, "It wasn't meant to be." "Better luck next time." "You weren't THAT pregnant anyway"....I know they dont mean harm, but how horrible. Those are so hurtful and such awful things to say.

What kind of treatment do you recommend for cases like mine? I read through the "Treatment" section of your site and I didn't see anything that really struck me inflammatory treatment-except for prednisone....is that all there's for cases like mine?

Is my pregnancy doomed to fail again given my labs? I had an ultrasound at 5 wks 2 days-only an empty gestational sac-my Hcg was 18534....I have very mild cramping now and brown spotting-still very light amount though. Is there any hope or is this probably ::another:: blighted ovum? :.(

prednisone/lovenox/Intra lipids/IVIG/ Neupogen are all anti inflammatory. Please call me so that I can help you.
