Yesterday (at 13w3d pg) I discovered my progesterone level is at 4.78. The lab rechecked and it came back low again and 5.1. Single digits at 13 weeks pregnant!

Throughout this pregnancy, I have been on 1.5cc of PIO daily and a 200mg progesterone vaginal suppository daily. At around 8 weeks pregnant, my progesterone level was at a 30. So good, but not extremely high considering the progesterone support I'm on.

My last PIO shot was at 12w5d (11-16-11). I've been continuing the suppositories. Yesterday (11-21-11) I had my local RE check my progesterone level b/c I was curious where it was at since stopping the PIO, and it's at a 4.78. My local RE and MFM had me start my PIO back up right away. I have an u/s scheduled tomorrow.

But I am freaking out. The placenta should have taken over long ago. However, with my history, I wonder if this hasn't been my problem all along...? I've had losses at 12w, 12w2d, & 14w. I know it's not the norm to check progesterone at this stage, but with my history, you can see how I would be concerned!

I just need to know what this could mean for me?? We're still hearing the baby's h/r on our home doppler, but could this have long lasting negative effects on my baby??? Could the suppositories alone save any damage this could have caused???

Ok I agree that you should start up the PIO , I would go to 800mg/day on vaginal supp. I have had cases like this, as long as you get back on track wtih your progesterone levels you should be fine. It would take some time with no progesterone to cause issues.

Hello Dr Braverman,
I just wanted to post an update on my progesterone issue. I immediately got back on my PIO shots, doing 100mg IM daily and 200mg suppositories twice daily. Since then I've consulted with another Dr who's switched me to 200mg of PIO twice a week and continuing the 200mg suppositories twice daily. So far, the baby has continued to grow. I'm 17w3d today. This week I also started 17 HP shots to help prevent preterm labor as I have a history of IC. So I'm probably on enough progesterone by now, I should hope.

Thank you for your thoughtful reply. I'm going to continuing all these progesterone supplements until it looks like my level is increasing on its own. If it never does, I'll stay on it until 36+ weeks, God willing.

Quick question...at what gestation in a pregnancy do you feel immune issues are no longer a problem? Ever?

your past that point, ususally at end of first trimester.
