Hi Dr. Braverman,I'm not sure if you remember me, my name is Heidi Seligman. 2 years ago was my latest success with you and Dr. Kofinas. Brief history short version: I have had 4 miscarriages sadly in total. I have had a loss , one before each of my three children. I have recently experienced my latest miscarriage
Now I know I sound selfish and everyone tells me I have three kids, (knock on wood), But we decided to try for a fourth. I moved to Orange County and tried to find a maternal fetal out here, but it backfired when he did not believe in any medicine that I usually take at all. I went to Dr.Kofinas at that point who thankfully called in all my usual meds , but it was to no avail. I missed at 7 weeks as usual, but this time Dr. Kofinas told my OB to send specimen for genetic testing..he was sure it was going to come back abnormal because the yolk sac was very big. I t came back normal female which was probably tainted from me. I have in the past been given (by you I had two successes, thank you!) Lovenox, aspirin , progesterone supp and then steroids for my last baby. Se was Breech and had low amniotic fluid so we took her out almost two weeks early and she was fine and weighed 5'14. my others were 8lbs! Anyway trying to make this quick! sorry! what is so perplexing and scary is that I always have had a loss before each baby. I dont know if you remember but I always each time got pregnant on my own, my first try and thats what made me calm down a bit, thinking I'd get pregnant again right away and that maybe all my body needed was a jump start before I can get a healthy baby. Now, since my loss, I have been trying to conceive a few months with no success..I know my age is advanced right now, I'm at 37. I did have a DNC a few moths ago, and that makes 2 in total. I had a c-section for the first time with my last success...
Dr. Kofinas' nurse Kristine has been helping me over the phone by sending me for blood work starting my 9th day , and then every other day to see if I ovulated to begin my lovenox, and progest. I am also for the FIRST time on Metformin, Metanx and aspirin daily which I never stopped taking for months now.
I have had intercourse when I am ovulating and beyond, so I expressed my concerns with the nurse and she said they may refer me to you again. What can be going on here?

Hi again. Thanks for your reply I did schedule an appointment with you for January in Long Island. Quick question...I just found out that I have a weak urinary tract infection tonight. I am still trying to conceive actively, and I believe I just ovulated yesterday. My OBGYN left and his nurse said they believe I should still take antibiotics. In the event I would become pregnant, in your opinion is this found to be dangerous? could it cause another miscarriage if I did get pregnant .Or another question in my mind is that can it prevent me from conceiving. I tried calling Dr. Kofinas but he is away for tonight, and I know you have more experience with conceiving .Can I wait? thanks,

Any urinary tract infection can decrease the chances for conception(although not greatly), but I would treat now , if you need antibiotics call my office and Ill call the script in for you. Bacteria activate receptors that create an inflammatory response that can cause failed implantation. Call me if you need me.

Oh wow! Thanks! The OB I have been seeing out here (Monroe, NY) Did call in Cipro and then changed his mind when I said I was concerned I guess and changed it to Bactrim. I hope this will work! I am going to take it right now . I knew you would have so much knowledge as you always do . See you soon and thanks again.

Please call me so we can discuss, you at least need a cytokine panel to decide on the prednisone. The third trimester issues were not prednisone related.

Hiedi , start the prednisone at 10mg then come in for the immune panel.

Hi Dr. Braverman, I received upsetting news today about my progesterone level. My first reading was 19, then i went up to 27.1, and as of Saturday, my level was 26. I am 4 weeks and 6 days today . I am on 100 mg supp. twice a day. The nurse at Dr kofinas said that (hes on business vaca) its not the best level right now, and i know from the past that when my levels start to drop it means miscarriage. Is there any hope at this point? with my last success I was at 50 already She told me to drink a lot and rest and call if I start bleeding or anything, and not to stress out but I am so upset right now. taking more progesterone would that be useless at this point? I have promertruim pills 200 mg from my last miss.

start the prometrium 200mg per night.
