Dear Dr. Braverman,
I would like to ask you about the treatment I´m getting at the moment.
We we trying to conceive for about 1 and half year and experinced 1 miscarriage at 7th week and 1 biochemical pregnancy. I was diagnosed high levels of NK cells (29%), positive ANA antibodies, autoimmune thyreoiditis,as well as anti annexin V IgM antibodies and heterozyg. Leyden mutation along with homozyg. MTHFR 667t.
I was recomended to take Lovenox, baby aspirin, thyroid supplementation and also Intralipid therapy, low dose corticoids (prednisone 5 mg) and 4000 IU of vitamin D. After this I got pregnant and since 6th week getting also dydrogesterone as progesterone suplementation (twice a day).
At the moment I started 9th week with no complications.
My questions are :is 4000 IU of cholecalciferol safe for my developing baby durin the 1st trimester (normal recommendation are much much lower),or should I rather lower it? (Next week i will have my vitamin D3 level checked, what levels wil be ok?).
The second question : how long should I continue in Intralipid therapy? I take 100 ml every 3 weeks, planned till 12th week (but I read that sometimes it is recommended till 24th week).
Thanks a lot for your answers in advance.
Best regards.
Emma D.

I would get the vit D level as planned and make a decision based on that.
I only prescribe IL based on immune findings that I follow , I never see the benefit of continiung past 12 weeks. If immune parameters warrant I may adjust prednisone or possiblly introduce other treaments , but I never make decisions without follow up testing.
