I was wondering whether you can comment on what determines the dosage of Lovenox a patient is put on for antiphospholipid antibodies? Is the dosage dependent on how many they test positive for? And does a dosage of 30 mg vs 40 mg really make a difference? ie could one say that a miscarriage was the result of that 10 mg difference in dosage?

Also, I hear some doctors say 1 shot a day is sufficient even in the event of a postive pregnancy test. Whereas others are adamant of two shots a day. What should one do when they get contradictory opinions? Should they err on the side of more over less or less over more?

40mg is my starting dose, if the patients are thin I may start at 30mg. I also take into account the full clinical history and the testing of levels for heparin anti XA to determine need for higher doses or twice daily dosing.
