Low LAD in Pregnancy

Hello Dr Braverman,

I am looking for your thoughts on my situation. I am currently almost 12 weeks pregnant after 4 IVF cycles.

I was tested in May this year for LAD and my results came back as:

Flowcytometry: Negative
(T-Cells) IgM+: 1.0 %
(T-Cells) IgG+: 1.0 %
(B-Cells) IgM+: 32.8 %
(B-Cells) IgG+: 2.7%

I then lad LIT four times between June and August.

My re-test came back as:

Flowcytometry: Negative
(T-Cells) IgM+: 10.7 %
(T-Cells) IgG+: 11.2 %
(B-Cells) IgM+: 16.7 %
(B-Cells) IgG+: 37.5%

I then went on to have another LIT at the beginning of this month and had my re-test last Monday.

My current results are even lower that my initial testing.

Flowcytometry
T-cells IgM+ 5.7 %
T-cells IgG+ 8.5 %
B-cells IgM+ 1.0%
B-cells IgM+ 1.0%

My question is how important is LAD in pregnancy and is there anything that may be preventing my response to treatment? I am on 40mg of prednisolne and I am having IVIG roughly every 3 weeks due to high cytokines.

I would very much appreciate any advice/thoughts on my situation as it is worrying me.

If you are getting IVIG you do not need LIT. IVIG simply passively replaces the blocking antibodies you are failing to make. sometimes LIT does not work, in which case, if you were not getting IVIG you could try donor LIT to stimulate your LAD.

Thank you, Dr Braverman, for taking the time to respond to my question.

Can I ask if you believe there is a point in pregnancy in which LIT treatment (or IVIG instead of LIT) can be stopped altogether? I am currently having IVIG every 3 weeks (I am 13 weeks pregnant) and I am not sure how long we can financially keep having IVIG.

unless you have had a late trimester loss , if the only finding is low LAD I dont beleive you would need the IVIG past the first trimester. I have not seen all your results and do not know your entire history so this is based on what you've told me. remember many patients with successful full term pregnancies , never have LAD identified.
