Neupogen WBC levels

Hi Dr Braverman

i was just wondering if you could reassure me on what you would consider high WBC levels on Neupogen. I am 20 DPO from an IVF cycle & my levels are high with HCG at 1667.0, Progesterone 307.20 & my WBC is 60.8.

My clinic want me to stop Neupogen for 4 days then re-start at half the dosage but I'm reluctant to do so as I am an endo sufferer & don't want to upset the cytokine balance too much.

I'm also on 10mg steroids & 2 Clexane a day. I'd really appreciate your comments on if you think it would be worth me just reducing the dose slightly or if you would not be worried at this stage?

what dose of the neupogen are you on? I would also need to know the diagnosis of why you are on it. Cleary you have to follow the instructions of the treating doctor I can only tell you what I would do in my patients with similar issues.

Hi Dr Braverman

I am on 1mcg per kg & I weigh 73kg. The reason I am using Neupogen is 2 miscarriages. One missed miscarriage at the 13 week scan, heartbeat had stopped at 10+4 & then a 2nd miscarriage at 5 weeks. Then we couldn't fall pregnant for years & have just had our 1st positive in 8 years using Neupogen for the 2nd time but IVF cycle number 6.

I'm not going to stop the Neupogen I'm just going to decrease my dose a bit so I'd really appreciate your advice as I think it's a new drug to my clinic so they are being ultra cautios!

did they examine you and check your spleen? If all ok I would then just decrease the dose in my patients but would not stop it. Again you must follow the advice of the doctors who are caring for you as I do not know your case. Are you in the States or overseas?

I am in the UK but my clinic is in Greece. I'm feeling fine in myself & have lowered my dose this morning so will get my bloods done again in a couple of days & see what the WBC is. Normally my WBC sits around 35 so I'm not sure what caused it to shot up so high.

Ok please let your clinic know what you are doing, if this is the clinic in Athens I see many ot their patients for them and help to manage these cases throughout the first trimester. It is important in these cases to understand the "reason" for the losses so that adjustments can be made during your first trimester and appropriate follow up testing can be done.
