Hi Dr. Braverman,

I am going to a new RE and for some reason they have just implemented a new policy whereby they forbid the use of intralipids. The stated reason is that allegedly there is some new study showing that intralipid use decreases implantation rates. I hate to be a skeptic, but I do not believe them. I thought that I would ask you if you have heard of any such study. Thanks.

ITs not a new study but in fact an old study , I beleive the control group were all women over 40 and there was no immune testing on these women to decide who did and did not need the intralipds.
So in other words it was a study done by doctors that have no backround in immunology so they had no way to determine who should or sholdnt get intralipids so they just gave it out randomly. If I did that I woudl be out of business very fast.
IL should be used in patients with elevated NK activity with a history of RPL and in whom there are no other immune issues that require other treatments as well. If you dont divide the patietns that way in your working in a group that alrady have a difficult time becasue they are over 40 you will come to the wrong conclusions as they did here.
Of all the therapies I use IL is certainly the most benign in that its good at only one thing lowering NK activity. But if monitored correctly and patients not oversurppressed, if it did lower the pregnancy rates none of my patients would be pregnant successfully after failing so many times previously.
I hope this helps.
Also please realize that for doctors that have no backround in reproductive immunology , they have no choice but to adhere to these types of studies and have no ability to know why they reach wrong conclusions. They are just doing what they think is best for their patients. Any one of them that has called and spoken to be has always felt differently afer we speak.

Thank you Dr. Braverman. I really appreciate you taking the time to answer this question. My NK cells are slightly elevated at 15.9 so I would still like to use intralipids.

honestly that is such a borderline number , Im not sure ILs are critical, would need to know the rest of your testing to draw a conclusion on this.
