Dr Braverman....

We are in the process of setting up a consultation with you. My wife recently turned 40. We are making good quality embryos. She has a retroverted uterus, no scarring. Over the last 7 years we have experienced at least 7 early miscarriages. Some of them were chemical. In the last year we have attempted 3 fresh cycle ivf and 1 fet. The Fet never achieved a pregnancy, but the three IVFs all achieved pregnancy.

In the first ivf we had slow rising hcg, but did get a low hb at 50. Shortly thereafter we miscarried at 5.5 weeks. The only thing we took was progesterone supplementation. Hcg peaked around 10000.

In the second IVF we had multiples. Hcg didn't rise correctly... And it was thought we were losing one of them. We again attained hb in one of the fetuses at 106, but miscarried shortly after at 6 weeks. The other 2 sacs showed material, but significantly behind. D&C/karyotyping showed a normal male. We took progesterone supplementation, and lupron after abnormal hcg rise on this cycle. Hcg peaked around 24,000.

In our third ivf we achieved a singleton pregnancy. Hcg Rise that was perfect to 70,000+. We got a 116 hb at 6 weeks. At 7 weeks it was 115... And miscarriage shortly thereafter. It was determined that the fetus grew to roughly 6.4 weeks. This cycle we took wobenzym, baby aspirin, fish oil, 3 LIT (2 before cycle and 1 after pregnancy), folate. We have not done a complete immune panel, but have done the LAD test as required by the doctor in Mexico for LIT. We never checked whether LIT moved our LAD numbers, because our RE really doesn't beleive in that test.


My question for you is based on this history.. Is it pretty clear that we either have a clotting or an immune issue ? Do you test for both immune issues as well clotting? What about hla/dq testing as well as testing for mthfr. Our re dd not test for mthfr since my wife's homocysteine levels were normal.

Also.. We are scheduled for D&C on weds. Is it important to do some test right now?

I am very sorry for your losses. Yes we test for and specialize in the treatment fo every known cause of recurrent pregnancy loss. Not sure why you had LIT without a full workup but there should be no harm and still most likely will help , but we weill repeat the LAD for you to check on its status.
