Recommended autoimmune testing

Dr. Braverman,

I am 30 and have been recently diagnosed with unexplained infertility after 1.5 years TTC with no positive pregnancy tests to show for it. My husband's sperm analysis was stellar.

My initial testing reveals only elevated thyroid antibodies. I have been hypothyroid for over 10 years, and I am currently regulated on 88mcg Synthroid and my most recent TSH level was .4.

I also have vitiligo over 90% of my body, which first appeared when I was 6. 10 years ago, I had a strange illness that lasted ~1.5 weeks. It included severe intestinal distress (all food/liquid passed straight through me). As the intestinal issues were clearing up, one of my knees became very swollen with fluid, and I could not walk properly for several days. And immediately following that, I contracted conjunctivitis in both eyes.

The PCP that I saw at the time did several tests looking for bacterial/virus/parasite issues causing the intestinal problems, but nothing was ever found.

Approx. 4 years ago, a joint in my hand (thumb) became swollen and very painful for several weeks with no apparent injury causing it. I could barely move my thumb. I was tested for RA at the time, but it came back negative. It eventually resolved on its own and I have not had a recurrence since.

Approx. 2 years ago, I began having intestinal issues again. Most often with dinner, I would have severe diarrhea about an hour after eating. This lasted on a regular basis (at least once a week, oftentimes more) for nearly a year. I was tested for celiac during this time, but the test was negative. These issues finally subsided and I have not had anything similar for some time.

I'm sorry for all of the detailed information, but my question to you is what additional immunology testing beyond the normal fertility panels would you recommend? Is there anything in my history that jumps out at you? We are beginning our 1st IUI cycle soon, and I want to get more answers than I currently have. Thank you so much!

you certianly have signs of some inflammatory condition as well as autoimmune(thyroid antibodies). Your cytokine levels should be measured as well as NK , NKT and other activated T cells. Total IGG antibodies should be measured as well. I would be happy to assist you in this workup. Feel free to call my office and ask for Kim she will schedule a consultation with me.
