Safety of continued steroid treatment late in pregnancy to prevent miscarriage and early delivery

Hi Dr. Braverman,

I am currently 22 wks and have been prescribed 5mg prednisone since the beginning of my pregnancy to prevent miscarriage and combat against NK cells. This was prescribed in response to a successful 10 week early delivery in my last pregnancy. My doctor has prescribed this course until up to 36weeks however I'm still a little concerned about potential impact on the baby given most women come off it at 12 weeks. Am I right to be concerned or is such a low dose harmless?

I think that is a low dose of prednisone so risks would be minimal, but after 24 weeks prednisone can cross the placenta, so I would not prescribe to my patients unless I had a full immune profile and evidence from the TESTING that it was necessary.
