Struggling to find hope and answers- 2 possibly failed ivfs

Hi Dr. Braverman

We are desprate for answers and for our miracle baby...

We've been trying exactly for 1 year this week. I'm 36 and my husband is 34. I have a blocked rt tube and my husband has 40% mobility,3% morphology. He also takes tegretol and we were told it won't effect fertility. So far we have completed 2 ivfs with a huge factory in Melville ( wink, wink ; where I feel like a number and my dr wouldn't recognize me if I was standing in front of him).

Prior to my ivf I had a irregular water sono and had 2 polyps removed 5/31. My 1st ivf started 6/3/2013 I was labeled as a slow responder to my 225 of gonalf and 1 menopur.. I was upper to 2 menopur and all meds used IM. Ivf#1 resulted in 8 mature eggs, 1/2 icsi only 3 fertilized. We transferred 1- 8 cell "perfect" embie and a 6 cells b/c grade. Result- big fat negative.

Ivf #2 8/3/13 the pill for 3 weeks, 225 gonal f, 225 of menopur all IM. Results 6 eggs,4 mature, all icsi, 1 fertlized. Again I was a slow responder. I had my retrieval on Sunday 8/18. Today I received the devastating news 1 made it "but we won't know the quality it's too soon" grrrrrr. Obviously, I am feeling hopeless for this lonely embryo. My transfer maybe is Wed.

I feel my dr is missing something and I know, I am looking for the concrete answers of why /what is wrong with us. All I receive is vague general answers.. Being type A that doesn't fly with me. I have full coverage at my current dr being a teacher and all... I guess you get what you pay for. Sorry for the vent. Can you shine any light on our situation??

The first thing you have to do is undersand why the poor response the meds. With a quesiton of a blocked tube and poor response I would need to rule out endometriosis, whether or not you have symptoms. yes you failed transfer of a good quality embryo but it is hard to know if it really was gentically normal. needless to say you should do a complete immune workup to be sure that this was not an implantation issue before any other good embryo gets transferred. As you mentioned you do need to be "obsessive" with every single fact before you transfer any embryo. You have a few things here that need to be looked at.

Endo is something I always thought of but never inquired about. I've always had heavy painful periods until my polyps were removed in 5/13.

As for a poor responder my E2 levels take forever to rise after being on bcp which I despise.
Ivf#1 my e2 was 642 on the trigger day, it went to 1168.
Ivf#2 my e2 was 540 on the trigger day and went to 846.

I made my appt with you in Oct ( the earliest appt). Of course my husband and I are looking for a second opinion and answers. We are concerned that you are not a participating dr in my empire plan.. United healthcare- state employee... We currently don't have any out of pocket expenses with my current Re bc it's a " center if excellence". We are neverous about the costs but are looking forward to working with you.

Even though we are out of network wtih Empire we are contracted with them so our patients still do not have any out of pocket expenses(you can call and ask for Judy in our office who can explain this to you).
