Hi Dr. Braverman-
I recently became pg w/ IVF#1 w/ immune treatments (LIT, IVIG, Dex, Syntroid, Metformin). Although beta numbers were great, at the first ultrasound (7weeks), we discovered only an empty sac and had a d&c several days later.

All my immune tests were great while pregnant. Tests were done at 5weeks. However, my cytokine ratios were low (TNFa = 7.7 and IFNg = 3.8). I was on 1mg of Dex. Without any immune drugs, TNFa= 23 and IFNg = 8.

I'm guessing Dex lowered th1, but if I need to be on it for Ana+ (1:160 both before the use of Dex and while pg on Dex), how do I get my cytokines up? Do you recommend probiotics? If so, which kind? I'm confused about whether probiotics are helpful or not with my condition.

Thank you so much for your time.

I am very sorry for your loss. You should not be on any steroid wtih low cytokines, clexane alone is fine for the postive ANA. We are working right now on a blog about probiotics for both issues , low or high cytokine production. But yes I think probiotics are a great choice for this. I would recommend Align but without the B infantes.

I would be happy to consult on your case seems the management is a bit all over the place. If you need me feel free to call my office for a consultation.
