Thyroid Antibodies and DHEA?

Doctor Braverman,

I am preparing for an IVF. Test results have come back fine apart from my anti-tpo which is 22 units per ml (norm less than 5.6) and anti-tb 21.2 (norm less than 4.11). I give the norms cos this is from a clinic in Eastern Europe and perhaps uses different ranges. The doctor has recommended a tsh test and thyroid gland ultrasound and a visit to an endocrinologist and to hold on treatment for now.

While I wait for these tests I am trying to work out what is going on. I do have raised NK cells but I have known about these for some time and my thyroid function has been fine in all previous tests. The only change in last months is that I have been taking DHEA (25mg 1st month, then 50 mg for 2nd month and now 75mg in the last days). Could this have caused the problem?

I'm frightened I am not going to be able to do IVF because of this and would appreciate your thoughts.

new studies have shown that DHEA may activate the immune system so may not be best for those with immune issues. Thyroid antibodies do require a workup but even if sonogram and TSH testing is normal this could still result in issues with implantation.

Thank you Dr Braverman. So I will stop the dhea. Do forgive me but I don't understand what you mean when you say that 'thyroid antibodies do require a workup but even if sonogram and TSH testing is normal this could still result in issues with implantation'. Does it mean that I have no chance of going ahead with ivf that I will always run the risk of implantation failure? Is there not anything that I can take?

the antibodies can attach to the invading trophoblast even if the TSH is normal. I dont know your case so I cant make recommendations , but in my patients we would use lovenox for this.

Thank you. Is Lovenox something that you take around the time of treatment? I assume so. And should I stop my dhea?

I cant recommendations on your treatment with out a consultation as I dont know your complete history , feel free to call and schedule one.
With my patients with similar history I have had them stop the DHEA and start the lovenox at the time of embryo transfer.
