Dear Dr. Braverman,What is your standard treatment for APs and previous abortions and thrombosis?Do you have any experience whether intralipid positive effect on the APS ? Can it bind Antiphospolipid antibodies ?What do you think about hydroxychloroquine? I should start it before pregnancy and continue in pregnacy.

Best treatment for APS is heparin or lovenox (clexane) . No , Intralipids are not of benefit in APS and do not bind antibodies. You may be thinking of IVIG here. Plaquinil is good for antibody mediated issues. I I would not recommmend any therapy however unless I was familiar with your case.

Thank you for your quick response. I had two deep vein thrombosis under contraception. Two years later I have two missed abortion under heparin and ass. Both pregnancies stopped in week 8 and 9. Both have a heartbeat. In 2013 under IVIG, Granocyte, 20 mg Prednisolon and Quensyl I reached 32+5 week. My son had to be brought by Caesarean section because placental insufficiency. All this time before, in and after pregnancy there weren`t any antiphospholipid antibodies positive. Just ANA`s between 640 and 1280. Now, two years after pregnancy there are:ß2 Glycoprotein: 82,80 (IU/ml)

Anti-Phosphatidyl Inositol 82,80 (IU/ml)

Anexin V 8,7 (U/ml)No DQa and HLA similarities between me and my husband, no activation of NK cells as measured by the K-562 taget cell blood test. ANA actual 160. No missing KIR-genes.

When should I start Plaquinil and may it lower the antibodies?

Your work up and evaluation is still highly incomplete to make an evaluation. Please see our testing page and sample analsysis, this is what you need to make correct clinical decisions. I can tell by your testing and comments that the evaluation is quite outdated.
