Hi Dr. Braverman,

I was diagnosed with hypothyroidism while doing IVF back in 2013. Saw an endocrinologist and was put on synthroid. Only my TSH was high, all other levels (t3, t4, antibodies) were in normal range. I got pregnant and had a child in 2014. 7 months post partum I had my Thyroid levels checked again and my TSH wa 86 and I tested positive for antibodies. Endocrinologist told me it was likely an episode of post partum thyroidosis. Currently trying doing IVF for #2, Thyroid levels still normal (with synthroid, antibodies have not been re-checked), and had a failed FET of a PGS normal embryo. Reproductive endocrinologist said just bad luck and likely sill an issue of embryo competence. Former RE (moved out of state since first child) recommends testing for NK cells. Is it possible to develop NK cells after successfully carrying to term? What tests would you recommend? Would adding prednisone help, in your opinion? Thank you.

Your RE is partially correct. You should do immune testing , but limited testing i.e. testing for just NK cells gives you very little information. A complete immune profile (see our panel on the home page) with a complete evaluation should help you to make a diagnosis. The Hashimotos is very suspicious for endo as is the unexplained poor embryo behaviour(see my blog on CRAMS) and silent endometriosis.
