Good morning Doctor Braverman,


I had 4 miscarriages at around 7 weeks. I am now 6 weeks pregnant after a FIV+PGS with 2 embryos. My immune and coagulation testing were all normal including NK cells in blood (tested when not pregnant) and I am not allergic to gluten. However, in all my pregnancies my WBC count was always very high (what made my OBGYN suspect some immune mechanism). In my 3 last pregnancies, I suffered SCH which always resolved upping my progesterone dosage before the pregnancy stopped.

For this pregnancy, my doctor prescribed prednisolone 20 mg, lovenox 0.4 (once daily), Baby aspirin, vitamin D. I am also on progesteron 400 mg*2 daily and estrogen 2mg*2. Last monday I had some cramps and bleeding (first brown spotting and then light red) then it improved. Yesterday after 20 minutes walking again cramping then some bleeding at night. My OBGYN does not want to change my appointment for the U/S (May, 2nd) and recommended rest and upping the dose of progesterone to 1200 mg daily.

What do you think? Should I stop lovenox and/or baby aspirin?

I can not give you advice as I am not your doctor. In cases like this I typically stop the baby aspirin and continue all else. I dont think the immune workup you have had in an attempt to define the underlying cause for your losses was at all complete. I dont think progesterone is "the"answer here. At this point the odds are you will do ok, but should you experience another loss you need to contact me and let me get you a complete evaluation. Good Luck!

However, do you think that the fact I had cramps and bleeding means that my protocol is not the most appropriate to my condition?

These are common symptoms so by themselves not terribly predictive.
