hi dr. Braverman,


im re-posting in hopes to get a response/opinion.


back history: my husband has poor morphology and I have hashimotos. We also are a DQ-alpha match and I have hla-y restricting alleles. First Ivf cycle I got pregnant with twins but had a subchorionic hematoma and continuously bled until my water broke at 16 weeks. Next 3 cycles were chemical and 1 cycle was a blighted ovum. We decided to try donor sperm and were successful first round and have a healthy son. i did have a subchorionic hematoma with ha pregnancy as well.

I was on prednisone, lovenox and intralipids.


2 questions: can we use the same donor or bc of my issues will I continue to miscarry now bc I carried a full term son?


is there anything I can do to try and prevent not having a subchorionic hematoma in a subsequent pregnancy?

you could certainly use the same donor. I think the SCH is a function of some immune issue but with out full immune testing we can tell you how to change the treatment.

Thanks for your response, Dr. Braverman. So, just so I understand correctly--having a full term son with donor sperm won't trigger an immune response where I'll have miscarriages?


and in general, what do you suggest for your immune patients to avoid subchorionic hematomas?

yes you are correct there is no risk by using the same donor. To try and prevent SCH you would need a complete immune profile and then determine the best immune therapy. I think with a succesful birth (regardless of the SCH) this expense may not be warranted.
