repeated low implantation pregnancies

I've had 2 miscarriages at 12weeks pg. Both pregnancies implanted low in my uterus. I've had a saline sonogram of my uterus and everything comes back normal. I'm currently 6w3d pg with twins and this pregnancy is implanted low in my uterus as well. Heart rates are good at 125 & 123 but my Peri is very concerned with the repeated low implantation. He also said the u/s shows a thick white wall around the sac...thicker then he normally sees at this point in a pregnancy and that that can indicate miscarriage.

With my previous losses, the baby is alive until my body aborts it. What would cause these pregnancies to repeatedly be low implanted? How can I not expect this pregnancy to end in a loss as well? Both miscarriages showed a chromosomally normal girl & a chromosomally normal boy. I've had bleeding with this pregnancy too.

I've also tested normal for all blood clotting disorders that cause a m/c b/c I developed a blood clot in my leg after my last miscarriage...all they found was a protein s deficiency. I'm on lovenox 2x a day for this pregnancy.

I'm looking for any insight on this issue. I'm very afraid I could lose another pregnancy at the 12 week mark...maybe sooner since it's twins. Thank you for your time.

As we spoke today, the most obvious reason for your late losses may be your Protein S deficeincy that is only now being treated with this current pregnancy. Hopefully this is all you will need. The lovenox recommended is the correct treatment.
The thick white ring around the gestational sacs may be a sign of an inflammatory reaction and therefore we should look at your immune profile to be sure there are no signs of immune rejection of the pregnancy. If there is any evidence that the immune system is involved this can still be treated so its not too late to investigate this.
The fact that both your losses tested normal for chromosomes indicates that the etiology of your losses is either thrombophilic(blood clotting) or immune in nature and many times both.

Thanks for your time Dr. Braverman. What are the risks associated with IVIg therapy? Would that be our next step if I did in fact have immune issues?

IVIG therapy has been used safely for over 15 years in our practice without a single complication. As a blood product you still need to be aware of the theoretical risk of blood bourne transmission of viral disease but the product is so thoroughly treated these are rare and again we have never had one.
After the immune workup we can determine which therapy is best for you , you dont necessarily need IVIG there are many other treatments . This will dependon how significant the findings are.
