Can Ivig and Neupogen be used during the same IVF cycle?

Hello Dr. Braverman,
After 4 IVF failure and many failed IUIs, my immunologist suggested me using the ivig before tranfer in my next IVF ( I had tried 6 months of ivig + natural cycle with no sucess), but my RE is strongly opposed to the ivig as he doesn't think it's working for me. He suggested using 60 mg neupogen from the day of transfer and if possitive, continue for another 40 days. I have read info related to both ivig and neupogen, but not seeing much information regarding using these two medicine in the same IVF cycle, usually people shared their experience of using either one, but not both. Would you be able to shed some lights? What is the reason why thest two medicine can't be used togather if that is the case. If they can be used togather, what type of patient would benefit the most from the treatment?

in experienced hands they certainly can be used together. It is more important to understand what you are treating. In cases where we suspect the cytolix T cell/ T reg cell ratio to be high we prefer to use IVIG or IL / even prednisone along with the IVIG. It seems from our data that the recruitiment of T regs to the decidua may be helped by neuupogen while IVIG raises the T regs but may not force their recruitment. These are things that we follow during the treatment phase to help us modify the treatment as we go along. Simply giving either of the treatments blindly is not the optimial way to use either durg, its more important to understand the diagnosis so these treatments can be tailored.
