Elvated TNF-a / IFN-g

Dr. Braverman,
My TNF-a (38.2) and IFN-g levels (21.0) are elevated...is it be best to lower these levels prior to a FET if all other immune tests are within normal ranges? Also, how would you go about treating when all other testing is normal? Thanks.

The problem with the elevated cytokines that you listed is that they make it harder for the embryo to develop immune tolerance in the mother and lowers the success rates. Eventually elevated levels will activate the other immune cells. In this situation I prefer to use prednisone as a treatment during the FET cycle.

How much Pred would you use in a case like this and for how long? Thanks you.

at least 20mg but I would need to see the remainder of my immune profile to give you definitive recommendations as this could change my thoughts.

How much Dex equals 20 mg of Pred? Also, does Dex and Pred work the same way and are both equally effective? I have heard that Dex crosses over the placenta, but Pred does not. Thank you for you time in answering our questions. You are appreciated.

In your reply to Lizib, you said that you would probably use 20mg Prednisone. How many mg of Dexamethasone would be equal to that? My doctor uses Desamethasone.

I dont like dex for this as it crosses the placenta but 90-95% of prednisone gets broken down by the placenta and does not get the baby. Most use dex for routine IVF as it is stopped will prior to fetal circulation.
