Intralipids vs Ivig

Hi Dr Braverman
i know a lot of studies have shown that intralipids works just as well as Ivig does in supressing NK cell activity. But I have read that Ivig is stronger than intralipids, is this correct? Do intralipids and Ivig work in different ways to protect the embryo? As I was reading on your. Webpage that Ivig acts like shield on the embryo? Does intralipids do this too or differently?

While some of the effects of Intralipids and IVIG may be similar(i.e NK cell deactivation and reduction in T cell cytotoxity , there are different mechanisms that acheive this and those mechanisms also give vastly different benefits in different immune situations. Intralipids have found very little use in TH2 (antibody mediated states) and those of elevated inflammatory serum cytokines. IVIG does have the "shield" affect you mention as one of its MANY benefits. IL do not have this benefit. IL is not a "poor" mans IVIG it is a completely different treatment with vastly different indications than IVIG .
there are not "a lot " of studies showing IL works as well as IVIG but this is simply not true when all variables are correctly controlled. IL is a good treatment when used in the proper situations. NK cell elevation is not a "diagnosis" its just a symptom and to determine correct treatment you must find the diagnosis that is causing the NK cell elevation .

Great Thankyou. So woukd you have situations where you woukd give your patients both IL and IVIG?

No , IVIG does all that IL does and a lot more so never a need to overlap.
