What to do and in which order?

Hi dear dr braverman
I have endo.etriosis in the left cyst 1cm. Left ivery occupied by a big cyst 7cm which has liquid in. Not endometriosis.
I am 41 years old a d i want to have a baby with my own eggs. Is it necessary to have a laparoscopy to remove the cyst? I should tell you that i ve already had 3 surgerues to remive endo cyst and now i am a low ovaruan reserve. . What to do next? 1 or 2 cycles of ivf freeze the eggs or embryos and tben put them in uterus after a surgery to clean endo?. If i di t remive tbe cyst with a laoariscopy and take visannette and then lut ebryos chances r lower? My ladt minths fsh is 11.46mIU/ml and antimulerian hormone 3.37mpol/ L. Pls let me kniw what to do.

this question is hard to answer without knowing the results and findings of each surgery. Did they not only remove endo cycst but excise all the endo he could find. If no more surgery then yes do a couple of cycles of IVF and freeze and if embryos still poor qualtiy then you can consider repeat surgery. Concerning my previous email, thread "What to do and which order" I didn't understand your answer at all! Can you pls explain again?


Previous surgeries were made to remove the cyst. The first time the biopsy showed normal cyst and the last 2 endometriosis. After each surgery I took arvecap/zolodex injections for 4-6 months to fight endo. I have know idea if they removed only the cyst from the ovary or clean all the endo they could see!


What matters now is that I want to have a baby with my own eggs! With low overian reserve and at the age of 41 i have no options for mistakes or lose time. I know my chances are very low, and this is the reason I contact you for your precious advice. I trust u a lot with all i ve read for you. Pls advice me what to do.


What is best to do maximise the chances for a baby? What options I have and which you recommend, although you did not examine me and don't know all my history.


Brief of my now medical situation: I now have a big cyst on the left ovary 7cm diameter (7*7*6). 3 doctors say it is not endo. This cyst contains liquid and it was aspirated 3 times but it fills again. My doctor says it is endo. Also a small cyst on the right ovary that I found about a week ago which looks like endometriosis. This is a small one only 1cm diameter. No other gynaecological problems as far as I know.


If laparoscopy my fertility will reduce even more. If not, the endo's impact on my eggs and implantation will be negative.My ovaries tend to create cysts even after 3 surgeries, so even If removed it will appear again!


OPTIONS I think i have

Aspirate the left ovary's liquid in which phase? before starting IVF injections? or ?

1.Take a few cycles of IVF, freeze, laparoscopy to remove endo, then treatment for endo with injections to stop period for how long?, then embryo transfer

2. Take a few cycles of IVF, freeze, if no laparoscopy what treatment for endo and for how long?, (i.e visannete? what else? then embryo transfer

3. Laparoscopy first and then IVF cycle but lower ovarian reserve


Any Option you recommend???


My FSH was a bit high so I took DHEA to improve egg quality and also drop my FSH before the IVF. IVF succeded from the 1st time and I got a healthy baby in 2015. No pregnancy problems at all.

Do you think that your supplements can benefit me better than DHEA? Do they drop FSH?? DHEA are famous for improving egg quality and they worked for me.

I ve already thought to order your supplements. For how long shall i take them?
