What to try next?

Hello Dr Braverman, I'm at a loss and have only just heard of you, so am really hoping you can help.

I am 33 years old, the main issue for us getting pregnant is my husband's sperm, especially his high levels of anti sperm antibodies.

We have had 5 fresh cycles of IVF (ICSI) and 3 frozen ones. Every time we get amazing embryos, last cycle they thought I was an egg donor my eggs were such good quality. My lining always looks perfect. Then it all goes wrong, I often get to implantation (I know because I can feel it and often pass tissue after), occasionally get chemical pregnancies or nothing at all.

I have been pregnant naturally when I was 18, I had a termination as it really wasn't the right time for me to have a baby. That was with a previous partner. Myself and my husband have been trying ICSI and naturally for 8 1/2 years.

We are at a loss, every clinic tells me it will work each time and they don't know what else to suggest. I have intralipids and prednisolone, as well as clexane.

My clotting tests were just below the normal range, I mean 1 or 2 out.
I do have anti thyroid antibodies and am on thyroxin, my TSH level rises when I do an ICSI cycle, but only to about 2.7.
My NK levels have been raised in the past, put intralipids seem to lower them.
I have had a hysteroscopy, all was fine.

We just don't know what to do next. Your advice would be MUCH appreciated.

Hi Angel, do not despair, I have seen many cases with your history and have had success. Best would be to call my office and they can schedule a consultation for you to give me a chance to review your case in more detail. I do not think this is related to the anti sperm antibodies.

I have booked an appointment with you on 27th Dec.

Will you be able to advise me what drugs/things we can try next?

It will be difficult to cycle with you over there, but I would really like your advice.

We do not collaborate with outside centers anymore. But lets see what your issues are and maybe we can still work out a way to follow you.

That would be good. I cannot cycle over there every time as I live in the UK and am a teacher, so getting time off is not easy. I can't quit my job or I won't have any money to pay for treatment, so it's catch 22.

An immune protocol and your advice will hopefully give me that much needed piece of the puzzle. After 8 1/2 years of trying you are one of my last hopes, so am over the moon you can still help.
