Accurate identification of the cause of a patient's multiple miscarriages or recurrent chemical pregnancies is vital. At BRI/ESSI, we have in-depth understanding and experience with diagnostic testing. Whether you are experiencing unexplained infertility or recurrent pregnancy loss, accurate testing is imperative. The field of reproductive immunology provides some impressive testing capabilities, and our team has contributed refinements to these methods.

There may be non-immune causes of miscarriage or failure of an embryo to implant, or immunologic causes. The immune evaluation considers three situations:

• Pre-existing inflammatory conditions such as autoimmune disease, endometriosis, or Polycystic Ovarian Syndrome (PCOS) in the mother.
• Immune rejection due solely to the failure of the embryo to generate immune tolerance, with no pre-existing inflammatory conditions. This may be due to genetic disorders or HLA matching issues between the embryo and the mother, the latter involving testing of both parents.
• A combination of these two options.

Our clinic utilizes thorough testing at all phases of a patient's treatment program in order to give us as much information as possible and allow us to monitor and guide conception and pregnancy. We conduct initial tests to determine an appropriate treatment program, then test again following treatment but before pregnancy. If a failed IVF cycle or miscarriage occurs, we perform specific tests to identify unresolved issues. Even after a successful conception, certain tests allow us invaluable insight into the progress of the embryo and pregnancy.

Treatment

Years of experience and extensive testing have consistently shown that, in most cases, the patient journey ultimately converges on a single course: a comprehensive surgical evaluation. After 30 years as a Reproductive Endocrinologist, our Medical Director, Dr. Andrea Vidali, understands the crucial role Endometriosis can play as an underlying condition causing multiple failures and pregnancy losses. Through expert excision of Endometriosis or correction of structural abnormalities, we’ve helped women who previously suffered repeated pregnancy losses achieve great success, carrying healthy pregnancies to term.

As such, BRI/ESSI has evolved to become an Exclusive Endometriosis Treatment Center, addressing the need for a hands-on and personal approach for the diagnosis and treatment of Endometriosis and the role it plays in infertility.

While we no longer offer immunological testing or treatment services, our experience in this field continues to guide our understanding of the hidden drivers behind reproductive challenges. In particular, we have seen time and again how underlying conditions such as Endometriosis and Adenomyosis—often inflammatory in nature—play a critical role in reproductive failure and frequently go undiagnosed without surgery.
